Add copyright notice and commercial licensing information

This commit adds a copyright notice and commercial licensing information to the source code. The copyright notice specifies the license under which the software is distributed, while the commercial licensing information provides details on obtaining a commercial license for using the software without the copyleft provisions of the AGPLv3.
