Detect and switch between multiple instances of an application

Summary

Some applications run multiple independent processes or instances rather than exposing all content as windows of one process. Reef may detect only one instance, leaving the others unreachable from the switcher.

Investigate process and bundle identity handling while preserving fast window enumeration.
