Fix CSV file formatting #744
Description
Please validate the CSV files for correct formatting using https://csvlint.io or some other tool of choice before publishing. The source_documentation_reference in the https://github.com/OHDSI/CommonDataModel/blob/main/inst/csv/OMOP_CDMv5.4_Field_Level.csv file is missing quotes.
