Denial clarifications

Clarify how denial metrics should be aggregated

• TR denials must be reported by Title.
• IR denials must be reported by Item.
• DR denials to the WHOLE database are aggregated and reported against the database Data_Type (e.g. Database_Aggregated).
• DR denials to items or titles within the database are reported by the item's Data_Type.

For books, denials accrue in the same way as usage metrics (Investigations and Requests): Attempting to download the whole book should result in 1 No_License for each chapter.

I think we need to clarify that the Denials in DR should be counted when access to the database is denied. When a user tries to access full content included in a database and does not have access to the database, that is a No_License for the database in DR and for the title in TR and/or the item in IR. If the user has access to the database, access to a full content included in that database should never result in a No_License (maybe a Limit_Exceeded for the database). If the full content is just linked from the database, but not included in the database, that should not result in a No_License for the database in DR, but only for the title in TR and/or the item in IR.
