International Orders Outside US, Canada, Australia or Manual State/Province Method Needed

The code should provide a method for manually entering states/provinces for areas outside of the US (or US, Candana, Australia once that issue is resolved.) Right now, any other selected country empties the state/province drop down, which is a required field, and there is no way to enter the information manually.

Clarifying proposed fix/feature request:

• If the selected country is US, maintain existing functionality allowing for drop-down selection of US states.

• If the selected country is NOT US then provide a mechanism for allowing text input of the state or province

Issue #117 doesn't really solve the major problem (i.e., even if #117 were solved even basic orders from European countries, Mexico, etc. would still be unsolved.)
