[Configuration] - within optimal range window & Use Supplemental Session Status

In configuration, when the field "Use Supplemental Session Status" is set to No, the default is that this option appears as a big No on every visits for within optional and within permitted.

What did you expect to happen?
A clear and concise description of what you expected to happen.
These two fields should not appear in the visit if No is selected in the configuration module because now it looks like the the data is problematic when it isn't.

The decision for this is as follows:

1. remove any association between the "SupplementalSessionStatus" configuration and the optionmal/permitted functionality.
2. Make the columns show only when the visit_windows table defines ranges for the loaded visit
3. the yes no values are then computed based on the ranges defined and the values for this session.
