Support merging chapter parts

If a site has a post split across multiple posts, support combining them into one chapter in the epub, so this would only affect make-epub.py

I guess it would ignore the title of all subsequent parts after the first, and just shove the content together with a cat type operation

Maybe something like:

• transform would need to be processed Last, and be skipped if the file to be created already exists
• Probably don't error out if one of the files listed by transform doesn't exist
• but definitely print out a FAILED message if a specific one is missing a file and don't create it. Go ahead and exit as well?
• check for parts before downloading a url, so won't know if its good or bad otherwise
• so will have to always basename, and then check the basename to see if it is expected to be parts

blah, looks like there are several places that don't use a common scheme for posting pages, even for parts of the same chapter. so, have to make sure to support some kind of explicit combination of pages into a single chapter.

epub_parts shouldn't care about missing parts, so it can be a more forgiving spec
