Why is it that for many team members, including myself, there are two rows (one with github username and dh-id, the other with full name and dh-id), but this is not the case for all team members. Shouldn't this be cleaned up to geta more transparent file?

The interface suggests that there is a problem with "row 45" = "line 44". I do seem to see many spaces which should be tabs, but I tried changing some and this hasn't helped. Please take a look.

No. There are sometimes more than one row for the same person because people have been using several different git names, either because they did not think to set the corresponding configuration variable, or because they used several computers or reinstalled git on their computers. It is useless to indicate the name of the person in the first column if the person has never pushed a commit with that name. The point is to map names that do appear in git commits, not to create new one that have never been used.

There was indeed a problem in the file: you inserted spaces instead of tabs in a tab-separated values file, thus the column count ended up being incorrect, hence the error message: "should actually have 2 columns, instead of 1".
