Fix rollout approval, rejection, and validation workflow

Investigate and correct inconsistencies in the rollout approval/rejection workflow between the old and new UI

• If Launch is requested from the old UI, the expected Approve/Reject controls are missing there while appearing in the new UI?
• A rollout can currently be launched even when required Risk questions have not been answered, with the error only appearing afterward
• Recreate the expected launch sequence and make sure blocking errors are checked before launch
• Fix rejection messages reappearing after a previous rejection has already been handled? (unless that is the same in current UI)
• Change Approve/Reject styling from red/green to grey/blue
