Update the FHIR transformer accounting for the latest changes

Recently after CARE v3, there are quite a lot of changes done to the EMR models, FHIR transformer util is outdated which causes invalid data issue while sending data in ABDM. Bringing this up to date should clear out multiple sentry issues.
