ManagerDeviceMemberStatusElement Should not reference DistributedApplianceInstanceElement.

Note that this impacts provider/consumer annotations on the SDK for all the remaining elements.

We can add methods to return the DistributedApplianceInstance name and Id since those are the only fields we care about.
