Unable to delegate with passphrase protected Trezor Model T

Hi, I have successfully created a wallet with passphrase protected Trezor Model T, I do following steps:

1. Connect Trezor when prompted at the page
2. Enter PIN, enter password
3. Have to go thru entire address selection process over including naming the account
4. Select Delegate
5. Fill the proper information (wallet and baker id)

However I end-up at this screen

But Trezor does not ask me anything, wallet is unlocked, no action shows up on Trezor Model T, I have tried plug and un-plug... I have latest firmware... It simply does not ask for the passphrase...

doesn't work. enumerates wrong address. it grabs the 1st available, not the subsequent ones with balance
