feat(cmux): adopt visible-terminal protocol v3 projection

Summary

• Adopts the generated cmux v3 head/worker projection for Codex.
• Replaces stale inline-shell task transport language with task-envelope/report-authoritative protocol wording.
• Keeps the schema-v3 fullrepo/bootstrap fixes from the earlier CI PR line.
• Adds generated English trigger markers for routing parity.
