refactor: remove shadcn tooltip layer#788

Replace tooltip wrapper usage with native title attributes where the hint still adds useful context, and drop repeated visible-text tooltips. Remove the app-level tooltip provider and delete the unused shadcn tooltip components. Signed-off-by: Matt Toohey <contact@matttoohey.com>
