Adds a PHILOSOPHY.md to the repo root documenting the narrative and philosophy behind DESIGN.md: what problems it solves, how it approaches them, and how the specification evolves.
Covers the prose-first design of the format, why specific references carry more than adjective lists, how negative constraints work, and how the format is extensible through custom sections and keys.
