[codex] Judge QA P3 delivery regression

What changed

• Extend clean-install to generate a custom Judge QA workspace and run judge-qa, seal, build, and verify-package from the installed npm tarballs.
• Require Judge QA Core/runtime and checker-interactor reference files in the published package inventory.
• Add regression coverage for pre-Judge-QA standard/custom/interactive workspaces and published package contents.

Validation

• npm run check: 665 passed, 30 skipped.
• npm run pack:check: probhub 218 files, probhub-skill 5 files.
• scripts/check_clean_install.py --json: passed, including judge-qa and rebuild identity.
• phtest: nine-problem lint and L02 no-cache Judge passed; worktree clean.
• Skill validation and git diff --check passed.
