Summary

• Switch Radar refresh workflows to the built-in GitHub Actions token and keep explicit token-env as a local override.
• Refresh the upstream review queue and add first-run source-backed review, impact, and social failure records for [codex] Rename multi-agent v2 assign_task to followup_task openai/codex#25636.
• Document and enforce Chrome tab cleanup expectations for Publisher and reset-watch automations.

Notes

• X publication failed closed because Chrome/X page readback was blocked by another extension UI; the failed social_post/v1 record preserves evidence.
• Decodex server completed XY-696 via PR XY-696: Audit MultiAgentV2 followup_task compatibility in Decodex #168 before this PR was opened.
