XY-702: Lane control P1: add run control channel and runtime audit records

Summary

• Add per-attempt local run-control channel metadata persisted in runtime SQLite and surfaced in operator status.
• Add fail-closed run-control request resolution tied to project, issue, run, attempt, thread, turn, active lease, and channel ownership.
• Add local private audit records for accepted, rejected, completed, failed, timed-out, and fallback control action outcomes.
• Update lane-control/runtime/operator docs and regression tests for stale turn/run mismatch, missing channel, and active ownership checks.
