Align Decodex app pills and startup

Summary

• show unassigned/global operator active runs in the Decodex App operator strip so running lanes can still surface as pills
• rename the macOS bundle/display name from Decodex App to Decodex, producing Decodex.app
• move Decodex App's fallback/listener contract to 127.0.0.1:8192 with --allow-unverified-codex, and align staging tests/docs
