Add Elixir StateTest fetch workflow

• replace the Python StateTest fetch helper with an Elixir Mix task and ignore the downloaded official fixture corpus locally
• improve official StateTest case generation so duplicate expectation names do not fail at compile time
• document the Mix-based workflow for fetching and running official Ethereum state tests against eevm
