Harden runtime planning audit flows

Summary

• clarify context-reset handoff/audit state and avoid misleading default reset reasons
• preserve runtime plan and operational commitment links for negative group chat ids
• bind single and parallel worker launches to durable plan steps, including follow-up lifecycle sync
• expose plan-worker bindings in CLI, gateway, web dashboard, desktop dashboard, and websocket snapshots
• align worker tool contracts so requested tools remain within template capabilities
• resolve runtime action contracts when bound plan steps are started through parallel worker launches
