docs: meeting-notes MCP façade runbook with tool sequence

Summary

Expands apps/llm-mcp-reference/shared/workflows/meeting-notes.md into a concrete MCP runbook for meeting-notes.process: tool sequence (describe_server → list_entrypoints → validate_entrypoint → execute_entrypoint → render_execution_report), sample transcript, runtime-owned field contract, and optional Cursor/Claude client snippets. Smoke asserts required tool names are present.
